Hospitalization AID Fund

Terms and Conditions:

• 1. Must be a regular member
• 2. Hospitalization Aid Fund (HAF) must be paid upon membership and renewed on or before the anniversary year of his/her membership
• 3. Pay yearly contribution ofPh₱ 500.00.
• 4. HAF can be claimed if member-borrower was confined 24 hrs and above.
• 5. if no payment within the anniversary year and member was confined, no claims shall be given.
• 6. Claims would be within 6 months and shall submit billing statement or official receipt
• 7. The amount of assistance is as follows;


• a. CONFINEMENT - Ph₱ 5,000.00
• b. SURGERY (Minor and Major) - Ph₱ 5,000.00
